Client A had been an in-patient in the local mental health hospital for 30 years.
A was successfully placed into ISL Supported Living at the age of 58. Through our graded support offer, they have achieved an impressive level of independence in 3 years. Their package initially began with 2:1 support within a 24/7 supported living setting, which was safely reduced to 1:1 support in the second year. Client A is supported in an independent flat within the service, with access to care as needed.
Over the three-year period, we achieved safe and sustainable reductions in support, successfully reducing commissioned hours by 168 hours per week. Throughout this time, there has been a consistent decrease in incidents, and Client A has not displayed any challenging behaviour in the last 12 months. Since moving into ISL, they have continued to make significant progress in integrating into the community, developing positive and healthy relationships, and no longer requiring PRN medication. Most recently, Client A was supported to obtain their mobility car in July 2024, which has further increased their independence and overall quality of life.
